If the client is under 18 years old, this form must be filled out by a parent or legal guardian.

** If you are sick, please do not book an appointment! All paid deposits including paying for brow tint in full with be kept and not refunded if you arrive at your appointment sick. Please do not forget to wear a face mask.

** Do not forget to fill out Health Declaration Form upon setting up an appointment.
